Pick your path. Same spreadsheet either way.
HST customers can paste in one report. Everyone else can type in their most common codes.
For HST customers
Run one report, paste it in
1. In HST, run the FI3009 Net Revenue Analysis by CPT report for your 2026 dates.
2. Export it, keep the nine columns the spreadsheet asks for, and paste them into the 2 Paste FI3009 tab.
3. Enter your NPI on the 1 Your Center tab and confirm your state and county. Check the data period and Medicare payer names on the 3 Check Data tab, then open Results.
Set your date range and filter the report before you export.
For everyone else
Type in your most common codes
1. On the 1 Your Center tab, enter your NPI or choose your state and county, and set Volume source to Quick entry.
2. On the 2 Quick Entry tab, type your most common CPT codes and how many Medicare units you bill a year (for most procedures, one per case).
3. Open Results to see your estimate.
Quick entry is a ballpark based on published Medicare rates and covers only the codes you type in. Pasting the report uses your actual contract amounts.
How it works
1. Tell it who you are
2. Add your 2026 Medicare volume
3. See what 2027 means for your center
What the spreadsheet shows you
Your 2026 baseline
Your Medicare payments at 2026 rates, scaled to a full year (from your own report if you paste one).
Your 2027 estimate
The same cases repriced at the final 2027 rates, with the dollar and percent change.
What is driving it
National rates and your local wage index are shown separately.
Code-by-code detail
Your biggest gainers and losers by CPT code, so you know where to look first.
Questions about the calculator
Is this really free? Do I have to give my email?
The spreadsheet is free, and you can download it without entering an email address.
Does it work if my center does not use HST?
Yes. Enter your NPI or choose your state and county, set Volume source to Quick entry, then type in your most common codes and how many Medicare units you bill a year (for most procedures, one per case). HST customers can skip the typing by pasting in one report.
Is this a prediction of what Medicare will pay me in 2027?
No, it is an estimate. It holds your 2026 case volume and mix constant, reprices each procedure that has both a 2026 and a 2027 rate, and shows you the lines it could not price. Drug rates change every quarter, and some drugs and devices lose pass-through payment during 2027 (for example J7355, on June 30, 2027), so the spreadsheet also shows a lower-end figure when those codes are in your data. It assumes your center meets CMS quality reporting requirements. It covers Medicare fee-for-service only, not Medicare Advantage or commercial plans.
Does it send my data to HST?
No. It is an Excel file with no macros. It calculates on your own computer and only opens a web page if you click a link in it.
